Do you accept insurance?

Dr. Sweeney and Sloan Smith are currently Out of Network Providers with all insurance companies. Therefore insurance is not accepted as payment, instead payment is due at the time of service. All major credit cards, checks, and cash are accepted. You are provided a superbill with applicable CPT and diagnostic codes that can be submitted to your insurance company for reimbursement. If you are concerned about reimbursement, contact your insurance company prior to booking and inquire about their coverage for Out of Network providers and the applicable service (i.e., psychological testing, developmental testing).

Do you offer payment plans?

For evaluations, payment plans are offered if needed. Please contact
